Education: Fourth Year Family Medicine Elective Rotations
Thank you for your interest in participating in a USU Family Medicine 4th year elective rotation. This step-by-step guide should help you complete the process. If you have any questions, please feel free to contact us. We are standing by to assist!
Step 1: Find a location for your rotation and a site preceptor willing to supervise and evaluate you. The course catalog lists many of these, but there are new unique rotations constantly being created. If you do not see the rotation you are interested in, or you would like to create a new opportunity, please contact Dr. Saguil at asaguil@usuhs.mil.
Step 2: Complete the top portion of USU Form 1304. The bottom section of this form must be signed by your site preceptor. When the 1304 is complete, please fax this form to Office of the Registrar, ATTN: Ms. Gail Hewitt-Clarke or Ms. Xochilth Romero (301) 295-3545. Also, please send a copy to Ms. Cecelia Cummings at ccummings@usuhs.mil.
Step 3: Determine if there is an existing agreement between USU and your rotation site. In such an agreement does not exist, one will need to be created. This process takes several weeks, so it is good to plan ahead. Read the USU Agreement Questionnaire Instructions. Complete the USU Agreement Questionnaire and e-mail this form to agreements@usuhs.mil. The instructions for completing this form are very helpful. Here are a few pieces of information you will need in advance:
- You are the "USU Requestor." Please include your middle initial and an e-mail address that you will have uninterrupted access to during the process. You may use Dr. Saguil as the POC for the agreement: LTC Aaron Saguil, MD, MPH, Family Medicine Department, 4301 Jones Bridge Road, Bethesda, MD, 20814, 301-295-9473, Dr. Saguil.
- The "Other Party to the Agreement" is your site preceptor. You will need to obtain the same detailed information from your preceptor as that listed for Dr. Saguil above. Also, check with the preceptor regarding who will be the "Signatory" on the official agreement. This is often the Commanding Officer at the institution or his or her designee. Obtain this person's detailed contact information as well.
- Ensure that all parties listed on the questionnaire understand that prompt response to e-mail inquiries will facilitate a speedy completion of the agreement.
- The dates of your proposed rotation (e.g., 22 Nov - 19 Dec 2010).
- The complete name and address of the institution where your training will occur.
- A brief statement of the purpose of the training. In other words, the experiences you will have at the site and the goals you hope to achieve during the rotation.
- Finally, information regarding the cost of travel (e.g., flight, millage, lodging, meals) should be determined. Students often pay these costs themselves. At many of the sites, the residents are happy to offer you a place to stay to help defray costs. If there is another source of funding, you will need the POCs detailed contact information.
Step 4: Complete the top portion of USU Form 114. The bottom section of this form must be signed by Dr. Saguil. You can send the form via e-mail to Dr. Saguil. He will sign the form and submit it to the Office of the Registrar. Ms. Cecelia Cummings at ccummings@usuhs.mil will keep a copy on file if you would like one for your records.
Step 5: Assure that you give a copy of the evaluation, USU Form 650, to your site preceptor. This form must be completed and returned to Dr. Saguil. Alternately, it can be sent to Ms. Cecelia Cummings at ccummings@usuhs.mil or faxed to 301-295-3100 at the conclusion of your elective rotation.
